TEMPORARY VS. PERMANENT

Most of our positions are temporary. Some assignments last just a few days and others extend for several years. You remain on our payroll for the entire assignment. We are responsible for your wages, taxes, worker compensation, bonding, malpractice and applicable benefits.

Most clients prefer us to send our employees ready to work; they trust our judgment to select the most qualified professional available. On occasion, you may be asked to interview with the client before the arrangements are finalized.

Occasionally, our clients ask us to recruit candidates for permanent positions. If you meet their established criteria and have expressed in interest in permanent work, we will consider you for these positions, as well. Our clients do pay a fee for this service. You may not, under any circumstances, accept a permanent position at your assigned workplace within six months of your last date of work without our written consent.

GETTING AN ASSIGNMENT

When our clients call, we spend time with them discussing the job to be done so that we get a very clear picture of the person that will do it best. After we obtain the details, our Employee Classification System is accessed and appropriate professionals are identified. If you are one of these candidates, we will call you and discuss the position with you. We will share the client's identity and location, the job duties, the pay level, the expected duration, etc. You must then let us know if you are interested in the assignment. If so, final arrangements will be confirmed. If not, we will note your file and reactive it if other positions become available.

OUR EMPLOYMENT PROCESS

The Employee Portfolio that you complete requests you to provide us with detailed information on your educational background, work experience and interests. Along with your professional resume and the personal interview, we are able to develop a complete and accurate view of how your background and experience match the requirements of our client needs.

We will enter the information you provide into our Employee Classification System to ensure that your qualifications will be considered as job openings occur. No applications are filed or forgotten.

Pre-screened: All candidates are required to submit a resume in advance. Once it has been reviewed, a personal interview is scheduled.

Interviewed: You will be professionally interviewed and evaluated. We learn about your expertise, work habits, professional plans and personal dreams. We believe our employees must be achieving their own goals for our employment partnership to be mutually valuable.

Tested: Knowledge and skill testing are utilized as appropriate.

Evaluated: Completion of evaluation forms, after your client supervisor has had an opportunity to review your performance, allows us ongoing quality control.

Drug & Alcohol Screened: All clinical staff are tested for drug and alcohol abuse.

OUR EMPLOYMENT POLICIES

CHG is an equal opportunity employer. We afford affirmatively equal opportunity to all qualified employees and applicants for employment without regard to race, religion, color, sex, age, handicap or national origin. Our employment policies are in compliance with government laws and regulations. All employees are appropriately licensed and certified.

We reserve the right to modify or terminate any or all of our personnel policies, practices, procedures and benefits at any time without notice and at our sole discretion. It is our policy that all employee relationships are at will and may be terminated by us or by the employee at any time for any reason or for no reason.

OUR WAGE & SALARY POLICY

All positions are classified according to job requirements and responsibilities. Rates of pay are set to realistically attract qualified professionals commensurate with their skills and the prevailing wages of the area.

ATTENDANCE & PUNCTUALITY

Your job contributes directly or indirectly to patient care. Being late or absent is unacceptable. Do not accept assignments unless you are committed.

If an emergency occurs and you are unable to report at your scheduled time, you must notify our Administrative Office immediately. If the absence occurs on a weekend or evening shift, you are required to contact your assigned facility and our Administrative Office. Remember, a record of good attendance and punctuality is a factor in the granting of continued employment, pay rate increases, assignments, promotions and transfers.

TIMECARDS AND PAYCHECKS

Employees are paid biweekly. The hourly rate for general positions is at standard time for up to 40 hours per week and at time and one-half for hours over 40. The hourly rate for usual staff relief positions, such as RNs, MLPNs, CNAs and other designated positions is at standard time for 8-hour and 12-hour shifts and at time and one-half for hours in excess of scheduled shifts.

Employees who work on designated major holidays will be paid according to the policy of the facility to which they are assigned.

We use one form to both pay you and to invoice our clients . . . the timecard. Timely submission of supervisor-approved timecards are your responsibility. They must be received in our office not later than 12:00 Noon every Monday. If you are not certain that your timecard will be delivered in time, arrangements should be worked out with our office. There is a dropbox in our building for those of you who prefer to deliver timecards personally.

Paychecks are issued every other Friday. Checks cover all hours worked for the two-week period ending the preceding Sunday. If you prefer to pick up your check personally, be certain to notify our office before Wednesday of payweek.

Remember to always be clear-spoken and courteous.

PERSONNEL RECORDS

It is important that personnel records be kept up-to-date. Please notify our office of any change in name, address, telephone number and income tax deductions. Higher educational achievements or additional experience should be reported promptly.

SAFETY

Catalyst Healthcare Personnel is concerned with the working conditions for employees and safety for patients and visitors as well. You are responsible for reporting fire hazards, unsafe practices or conditions, malfunctioning equipment and any accident occurring on the worksite to our office. To neglect reporting such information postpones corrections of hazards and puts everyone in danger.

BENEFITS

Certain benefits such as health insurance, dental insurance, life insurance, credit union membership, etc., may be available to you at cost. Contact our Administrative Office for further details.

IMPORTANT INCENTIVE BONUS PLAN!

The responsibilities and stressors placed upon each of you, because of your chosen profession, are immense. We are proud to know you and appreciate that you work with us.

We continually seek ways that we can reward you for choosing Catalyst Healthcare Staffing as your employer. We try to offer you a variety of assignments so that you remain challenged and satisfied. We encourage both long-term and short-term work schedules so that you are in control of your life, choosing the facilities you like, the shifts you prefer and the number of days that work best for you. We offer generous pay rates and access to health insurance, dental insurance, life insurance, bonuses and other perks.

In an additional effort to reward you financially and to help us recruit quality registered nurses and licensed practical nurses to work for us at our many client facilities, we would like to propose the following bonus plan:

1. If you recruit a registered nurse or licensed practical nurse to work for Catalyst Healthcare Staffing, we will pay you a bonus of $1 per hour for every hour that they work for us including overtime (paid at the flat $1 rate) for their first 12 months. You will receive your bonus each pay; there is no waiting period.
2. You may recruit as many RNs and LPNs as you wish.
3. The applicants that you send to us must provide us with one of the cards enclosed with this letter, signed and dated by you; failure by them to do so means that you forfeit any referral bonus. If an applicant provides us with a card that has more than one signature or with more than one signed card, the referral bonus will be split equally. New cards will be mailed out with payroll checks or you may obtain additional cards at our office.
4. You must be actively working for Catalyst Healthcare Staffing to receive your bonus. If you leave our employment, are unavailable for shifts or are terminated, you will lose all rights to any remaining bonus.
5. We reserve the right to modify or discontinue this program at any time. However, once you have officially referred an employee to us and your bonus has commenced, you are entitled to its entire term as long as you are still actively working for us.

This is a very rewarding program! If you refer one nurse to us who works 80 hours per pay period, you will earn a pay period bonus of $80. If this same nurse works full-time for the entire year, your bonus would be $2,080. If you refer 10 nurses to us who work 80 hours per pay, your pay period bonus would be $800. If all 10 nurses work for the entire year, your bonus would be $20,800!

We recognize that you are a wonderful source for recruitment; you know the skills and work ethics of your fellow professionals. This incentive plan allows us to reward you for your loyalty to us! If you have any questions, please be certain to let us know.

Cordially,
CATALYST HEALTHCARE STAFFING
Terry Walker Kalfs, President

REFERRALS

Catalyst Healthcare Personnel is always looking for good employees. Sometimes we need them for short-term staff relief, sometimes for long-term assignments and, occasionally, for permanent openings. If you refer a qualified person to us and we hire them and they work at least 35 hours per week for six weeks within their first three months, we'll give you one hundred dollars, just to say thanks! They don t have to be consecutive weeks and you can refer more than one person. Be sure your referral puts your name on our application.

WORKER COMPENSATION

All employees are covered by worker compensation laws in the event of an injury or illness caused by or arising out of the course of employment. Work-related injuries and/or illnesses, no matter how minor, must be reported IMMEDIATELY to us. Failure to do so will result in loss of benefits.

UNEMPLOYMENT COMPENSATION

In the State of Michigan, eligible employees are covered for unemployment compensation. To be eligible to receive benefits, an employee must be out of work through no fault of their own, able and available for comparable work based on skill, experience and education or training and actively seeking employment. TIPS ON MAKING YOUR ASSIGNMENT SUCCESSFUL

Place yourself in the position of your assigned supervisor! Remember that good work habits such as reliability, punctuality, confidentiality and honesty are just as valuable as experience, credentials and education!

Keep your resume with us up-to-date. We and our clients often use it when evaluating your potential for another assignment. Your resume should be a summary of important facts. It should describe your major strengths and experiences in about one page. A list of references should be offered on request or attached. Include your full name , address and telephone numbers. Organize your information; categories should include education, experience skills, activities and references. Use specific dates, names, titles and descriptions. Don's forget to include new skills you may have gained while on other assignments with us.

During the interview and while on assignment, sell yourself. Don't just answer questions, give examples of your achievements and illustrations of your personal qualities. Stay positive. Don't shy away from past shortcomings and don't blame others. Watch your non-verbal style. Make eye contact, look poised. Remember that attitude and body language are very important. Many times employees with great enthusiasm and personality and work ethics are retained over those with more experience.

OUR CORPORATE MISSION

We believe our first responsibility is to the doctors, administrators and managers who use our services. In meeting their needs, everything we do must be of high quality. We must constantly strive to maintain reasonable prices. Our customer's orders must be serviced promptly and accurately.

We are responsible to our employees, the men and women who work with us. Everyone must be considered as an individual. We must respect their dignity and recognize their merit. They must have a sense of security in their jobs. Compensation must be fair and working conditions must be clean, orderly and safe. Employees must feel free to make suggestions and voice concerns to us. There must be equal opportunity for employment, development and advancement for those qualified. We must provide competent management and their actions must be just and ethical.

We are responsible to the communities, in which we live and work. We must be good citizens—support good works and charities. We must encourage civic improvements and better health and education. We must maintain in good order the property we are privileged to use, protecting the environment and our natural resources.

If you believe we are effective in our quest for excellence, let us know by utilizing our services and by telling us so. If you have suggestions for ways of improvement, share them with us, as well.
